Harbor Care is the new shared name of Harbor Homes, Keystone Hall, Healthy at Home, the Harbor Care Health and Wellness Center, and the Southern NH HIV/AIDS Task Force. Working as one, the programs of Harbor Care provide the tools people need to improve their lives. Harbor Care operates a Department of Labor-funded employment services program for veterans from anywhere in New Hampshire experiencing homelessness. Veterans work one-on-one with an employment specialist to find and maintain meaningful employment, and, if qualified, may also access our full range of veteran services including transitional housing, case management, temporary financial support, on-the-job training, and more. Services: Harbor Care operates Greater Nashua’s only Healthcare for the Homeless Federally Qualified Health Center, the Harbor Care Health and Wellness Center, offering in-person and telehealth services to residents throughout New Hampshire for primary care, mental health care including medication prescribing, Medication Assisted Treatment, outpatient and residential substance use disorder treatment, pharmacy, and dental services to those with and without insurance, using a sliding fee scale based on household income.

Programs designed for infants, toddlers, and pre-school children to learn the habits and tools they need to succeed in school and in life beyond school. We believe in helping every child reach their true potential. All Head Start and Early Head Start children receive ongoing developmental screening and assessment to address their ongoing medical, dental, nutritional, emotional and social needs. Individualized services are also provided to meet the needs of children with disabilities. Enrolled center-based children are provided nutritious meals and snacks.

Wilson Street Integrated Health – Offers a comprehensive care team to address patients’ medical, behavioral, and social needs. Services include; primary medical care, mental health care, substance use services, medication-assisted treatment, nurse case management, health education, assistance with food stamps, Medicaid, and disability applications.

CAPHR has partnered with Hillsborough County for our Community Gardens Program in Goffstown which provides up to 50 garden plots available to all residents of Hillsborough County regardless of income. The growing season each year closes at the end of September. Plot size: 25’ x 25’ Fee: A minimum of 75% of all gardening space will be reserved for low-income applicants free of charge. The remaining plots will be made available to other interested community members for a donation of $20.

Holy Cross Family Learning Center offers English as a Second Language (ESL) for beginners and intermediates for the growing immigrant and refugee population of the Greater Manchester. Holy Cross Family Learning Center is a place where people of diverse cultures from the City of Manchester, New Hampshire, come together to initiate or to continue their education. As part of this learning, they participate in helping to develop the Center as a place where family values are strengthened, community building is fostered, literacy is nourished, academic performance is raised, and diversity respected and celebrated. HCFLC seeks to respond to the needs of the growing immigrant and refugee population of the Greater Manchester area especially on Manchester’s West Side.

Home Base providers are a strong team of educators, case managers, social workers, and therapists, who work together to provide comprehensive support. All services are overseen by onsite, full time clinicians licensed by the state of New Hampshire to provide mental health and substance misuse treatment. Homebase also provides case management, education, and clinical counseling services following a DUI conviction address a broad array of needs through a personalized program designed to help you restore and retain your driving privileges.

The Manchester CoC provides systems of care to those who are homeless or those at risk of becoming homeless. Established in 1995, the continuum works to quickly rehouse families and individuals experiencing homelessness and to promote access to programs while minimizing the impact on people and communities.
